In response to requests received by the state office from local chapters looking for an evaluation tool for new and veteran members, attached is a model evaluation form developed by the National Association of Sports Officials during its 2002 conference. The form can be customized to fit a local chapter�s particular criteria for evaluating officials and is a good generic form which is applicable to all sports.
